Carnosine Trial Finds Selective Endurance Signals, Not a Broad Strength Boost

A new placebo-controlled clinical trial reports a mixed result for carnosine supplementation: some endurance-style tests improved in selected subgroups, but the study did not show a broad performance upgrade across every measure.

The trial randomized 299 participants to either placebo or 2 grams per day of carnosine-containing capsules for about 12 weeks. Researchers tested hand-grip strength, bilateral calf raises, a 2-minute step test, and gait speed at baseline, around 6 weeks, and around 12 weeks.

The clearest signals were narrow. At about 6 weeks, participants under 40 in the carnosine group improved on the calf-raise-to-exhaustion measure. At about 12 weeks, men over 40 improved on the 2-minute step test, with a trend in men overall. The researchers did not report significant carnosine-versus-placebo differences for the other physical performance measures.

Why it matters for lifters

Carnosine and beta-alanine are often discussed in gym settings because carnosine helps buffer acid buildup during hard efforts. This trial is useful because it directly tested carnosine in people, not just theory or marketing copy.

For lifters, the practical read is cautious: the data may fit short, repetitive, endurance-limited efforts better than maximal strength. It does not show that carnosine automatically raises grip strength, gait speed, or every gym performance metric. It also raises a subgroup question, because the reported benefits appeared stronger in men and in specific age bands.

What to watch next

Future studies should test trained lifters, compare carnosine directly with beta-alanine, and look at exercise protocols that resemble real gym work: repeated sets, short rests, high-rep accessories, sled work, loaded carries, and conditioning finishers.

Readers should also watch whether follow-up research explains why the signal appeared selective by sex and age group. Until then, this is a measured evidence update, not a reason to treat carnosine as a guaranteed performance enhancer.
