A new PubMed-indexed review suggests that older adults may gain more lower-body strength from heavier resistance training than from lighter or moderate loading, while bone-density outcomes may be less clearly separated by intensity.
The systematic review and meta-analysis pooled randomized controlled trials comparing high-intensity resistance training, defined as at least 70% of one-repetition maximum, with low-to-moderate-intensity training below 70% of one-repetition maximum. The analysis included 18 studies and 1,283 participants age 50 and older.
High-intensity training outperformed lower-intensity training for leg press and leg extension strength. The review did not find significant between-program differences for lumbar spine bone mineral density, femoral neck bone mineral density, fall incidence, or adverse events. The authors also noted that more trials with standardized training protocols are needed.
The practical takeaway is not that every older lifter should immediately max out. The review points toward the value of progressive loading when the goal is strength, but heavier work still needs appropriate exercise selection, technical control, recovery time, and medical or coaching oversight when risk factors are present.
Why it matters for lifters
For masters bodybuilders, recreational lifters, and coaches working with older clients, the review supports a familiar principle: meaningful strength adaptation usually requires enough load to challenge the target muscles. Very light work can still have a place for learning movements, managing pain, or building tolerance, but it may not be the strongest long-term option for leg strength.
The bone-density result is also important. Lifters should not assume that simply training heavier guarantees a better bone response than a well-designed moderate program. Bone health is influenced by loading pattern, impact, nutrition, medications, hormones, fall risk, and medical history, so the best plan may be broader than just pushing intensity.
What to watch next
Future studies should separate beginners from experienced older lifters, track women and men clearly, compare machine-based and free-weight loading, and report adverse events in a standardized way. Coaches will also want longer trials that show how heavy training can be progressed safely over months and years, not just during tightly controlled study blocks.
